Enrollment Information 2011-2012
Information requested for enrollment in the Carthage R-9 School District:
*Proof of age (medical records, baptismal records, passport, etc.)
*Student’s immunization records
*Proof of residency (rental contract, realty contract, utility bill, deposit receipt or other special
circumstances)
*Last year’s report card or other proof of grade placement (except kindergarteners, who need proof of age)
* Present address and phone number
* Parent or guardian’s place of employment and work phone number
* Emergency contact person (in case parents cannot be reached)
* Doctor’s name and phone number
* Student’s social security number (optional)
* Students planning to drive to school must register their license plate number and may secure a
parking sticker to park in one of the student lots. A sticker is not required if the student
wishes to park on the street.

MIDDLE AND ELEMENTARY SCHOOLS
All elementary students (Grades K-6) new to the Carthage R-9 School District for the 2011-2012 school year will be enrolled at the school they will attend from 8:00 a.m. to 3:00 p.m. on Wednesday, August 3, or Thursday, August 4. Persons with questions regarding boundaries of elementary districts may call Dr. Mark Baker at 359-7001. All elementary students and parents unable to attend these enrollment sessions should report prior to the first day of school to the principal’s office at the school they will attend.
Children who are five years of age before August 1 are eligible to enroll for kindergarten. Students/parents should provide proof of age, which may be a birth certificate or hospital record showing date of birth.
Students new to the district may not enroll until verification of immunizations has been provided to the school. Students previously enrolled whose immunizations are due during the summer must provide proof of those immunizations prior to receiving their class schedules at the beginning of the school year or attending class.
JUNIOR HIGH SCHOOL
Junior High School students who have not previously enrolled, should enroll as follows:
Monday, August 8 New 7th grade students 9:00 a.m. – noon
Tuesday, August 9 New 8th grade students 9:00 a.m. – noon
SCHEDULE & LOCKER ASSIGNMENT PICKUP
Students in 7th grade who enrolled in the spring may pick up schedules and receive locker assignments on Wednesday, August 10, 9:00 a.m. – noon.
Students in 8th grade who enrolled in the spring may pick up schedules and receive locker assignments on Wednesday, August 10, 1:00 – 3:00 p.m.
SENIOR HIGH SCHOOL
Senior high school students who have not previously enrolled should select their classes and complete schedules on August 3 or August 4, from 8:00 to 11:00 a.m. or 12:00 to 3:00 p.m.
Students who enrolled in the spring may pick up schedules and get their locker assignments in the Commons Area on the following dates and times:
Seniors 12 – 3 p.m. Monday, August 8
Juniors 8 – 11 a.m. Tuesday, August 9
Sophomores 12 – 3 p.m. Tuesday, August 9
Freshmen Orientation 8 – noon Friday, August 12
Freshman Orientation will be held Friday, August 12 from 8 am-12 pm at the high school. Students will receive information about the routines and procedures of the high school, including their schedules and locker assignments. All schedules will also be available to view and print off the IC portal after June 10.
Padlocks on hall lockers are optional but are recommended for security purposes. If a lock is desired, it must be a school-issued lock. A deposit of $5.00 is required from each locker occupant and will be returned to the student at the end of the school year when the lock is returned. Physical education locks are required and will be issued on the first day of school from the physical education teacher, who will collect the deposit of $5.00. This will be done on a semester basis in the physical education classes.
Students planning to drive to school must register their license plate number and buy a parking tag to park in one of the student lots. The cost for the tag is $2.00. A tag is not required if the student wishes to park on the street.
